NextPhase Navigators was created to help families through one of the most confusing and stressful administrative processes they may ever face.
Most families are not prepared for the amount of paperwork, follow-up, and detail involved in a long-term care Medicaid or waiver Medicaid application.
Our role is to provide structure, clarity, and steady support throughout the process so that families can stay organized and avoid preventable mistakes.
We assist with application preparation, document guidance, submission support, communication support, and annual redeterminations.
We do not provide legal advice. If an elder law attorney has created a Medicaid spend-down plan, we may assist with implementing that plan during the application process.
