Families dealing with long-term care decisions are often placed in the middle of a process they do not understand and did not expect.

A parent may be entering a nursing home. A spouse may need more care than the family can manage alone. A hospital discharge may be approaching. At the same time, someone must figure out paperwork, deadlines, records, and follow-up.

That is where many families get stuck.

NextPhase Navigators was created to help families move through the administrative side of the Medicaid application process with more clarity and less confusion.

Our role is simple -- we help families understand what information is generally needed, gather and organize documents, prepare the application, support submission, and stay responsive when additional information is requested.

We serve as a guide through a process that is often unfamiliar, stressful, and time-sensitive.
