Families often delay making contact because they assume they must first gather everything.
That is usually not necessary.
You do not need a perfect file in order to begin.
It is helpful to have basic information such as the applicant’s name, date of birth, current living situation, any facility involved, a general understanding of why Medicaid may be needed, and any deadlines or notices already received.
Depending on the situation, families are often asked to locate items such as:
- Identification
- Social Security information
- Health insurance information
- Bank statements
- Income information
- Asset information
- Life insurance details
- Property records
- Vehicle information
- Burial documentation
- Existing Medicaid correspondence
