What We Need to Get Started
Families often contact us before they have everything organized. That is perfectly fine. You do not need to have every document in hand before reaching out.
However, the following information is often helpful when beginning the process:
- Name of the applicant
- Date of birth
- Current living situation or care setting
- Name of nursing facility, hospital, or community program involved, if applicable
- Basic summary of why Medicaid assistance may be needed
- Names of family members or decision-makers involved
- Any deadlines, notices, or requests already received
Documents That May Be Needed
Exact requirements vary by case, but families are often asked to help gather items such as:
- Photo identification
- Social Security number or card
- Health insurance information
- Bank statements
- Income information
- Asset information
- Life insurance information
- Deeds, titles, or other property records
- Prepaid burial or funeral documentation, if applicable
- Any notices or correspondence already received regarding Medicaid
